community support team

Community Support Team (CST) is a comprehensive, community-based behavioral health service designed to help individuals with severe and persistent mental illnesses manage their symptoms and improve their quality of life. CST offers a range of services including crisis response, symptom management, coping skills, and assistance with social integration.  

The Community Support Team is comprised of four members: 1 Licensed Clinician, 1 Behavioral Health Qualified Professional, 1  Substance Use Counselor, and 1 Peer Support Specialist.

1. Services Provided: CST offers a range of services, including symptom management, coping skills training, assistance with social and community integration, and referral assistance with housing, education, and employment.
2. Team Composition: The Team is made up of 4 individuals: a licensed clinician, a substance use counselor, a qualified mental health professional, and a peer support specialist.
3. Settings: CST is provided in various settings, including the individual's home, community, or office. CST ensures accessibility to service, especially for those who may be homeless. This person-centered service is tailored to each individual's needs.
4. Availability: CST provides 24/7/365 crisis response to benefit those with serious and persistent mental illness by promoting symptom management, community integration, and reducing hospitalizations and the need for inpatient/residential care.
